New application form to request use of demand printed labels for specialist service referred samples

Apply to use demand printed labels for specialist service (RCI, H&I and IBGRL) by completing an online application form. Please make one submission per hospital / Trust.

Details of the label requirements are specified on the form.

The form is published on the RCI, and H&I pages on the Hospitals and Science website and on the IBGRL web site, in the documents and forms section on the fetal genotyping diagnostic page, the fetal screening page and the red cell genotyping page.

If we have already approved your use of these labels there is no need to submit a new online application.
