Use a second RCI request form to support verbal requests for additional crossmatches

Please email or fax this form to the RCI laboratory, after making a verbal request to add an additional crossmatched unit/s onto the original referral. This is in addition to the telephone contact.

This practice is already in place at three of our laboratories and we are now extending it to others as an additional safety measure, to ensure we meet the correct requirements and to avoid any potential incidents due to communication or transcription errors.
