Request for emergency delivery

Place your request using OBOS and then telephone Hospital Services to alert them. It is important you follow up the request with a telephone call.

The hospital blood transfusion laboratory must:

• complete the relevant sections of the emergency component request form (Word 49KB)
• fax or email the form to their blood centre
• telephone to confirm receipt, this call marks the start time of the delivery
• please confirm details such as target delivery time during the call 

In extraordinary circumstances we will issue components without a fax or email, but hospitals must recognise that mistakes are possible where written confirmation of an order is not received.

Without the confirmation telephone call there is a risk that these requests are not processed immediately.
