How to refer a patient

This page explains how to refer a patient for a TAS treatment.

Referrals for plasma exchange

A new online referral form is now live for plasma exchange (PEX) only.

Our online referral form is currently not available

We are working to fix this as quickly as possible.

To make a referral please follow these steps:

Step 1: telephone 0300 020 0496

Step 2: complete a treatment request form and in hours return to your closest TAS unit or out of hours to TAS.Referrals@nhsbt.nhs.uk

Referrals for other TAS treatments and locations

For all other treatments and units follow the instructions below to make a referral.

The steps you need to follow to make a referral depend on whether or not your Trust has a Service Level Agreement (SLA).

Trusts with an SLA

Urgent referrals

From Monday to Friday, 8am to 5pm, follow these steps:

Step 1: telephone 0300 020 0496, select the option for your nearest unit
Step 2: email a completed treatment request form (Word 55KB) to your closest TAS unit (please note, this link will take you to the NHS Blood and Transplant website for the public)

Outside of these hours, follow these steps:

Step 1: telephone 0300 020 0496, select option 1
Step 2: email a completed treatment request form (Word 55KB) to TAS.Referrals@nhsbt.nhs.uk

Non-urgent referrals

Email a treatment request form to your closest TAS unit (please note, this link will take you to the NHSBT website for the public)

The team will be in touch with you the next working day. Telephone 0300 020 0496 if you don't hear back the next working day.

Trusts without an SLA
