To record the details of your consultation with a hospital following a patient reaction during transfusion use the Record of medical consultation (Word 50KB) form.

Forms for hospital staff

RCI form 1A 

For the investigation of:

  • Allo and autoantibodies
  • Haemolytic transfusion reaction
  • Haemolytic Disease of the Newborn
  • Autoimmune haemolytic anaemia
  • IgA deficiency

RCI form 1A

H&I form 3A 

For the investigation of:

  • Transfusion Associated Acute Lung Injury (TRALI) 
  • Transfusion-associated graft vs host disease (TA-GvHD)
  • Post-transfusion Purpura (PTP)
  • Non-Haemolytic Febrile Transfusion Reaction (NHFTR)

 H&I form 3A 

Non bacterial infection report form

When a patient has a non bacterial infection post-transfusion use the non bacterial infection report form FRM1534
