A review of the H&I and PGI request forms has taken place. There are now five forms (3A - E) and these will replace the current forms 3a, 4a and 5a.
The forms are available to download from this site or for printed copies please contact you’re local H&I laboratory by telephone. For general enquires regarding H&I services please contact us using the following email address HINatnl@nbs.nhs.uk.
The re design of the forms was made to be more user friendly, collect the relevant clinical details to enable us to perform the appropriate test (s), to reflect the services that are currently provided by the laboratories, to comply with accreditation regulations and in response to user feedback. |
3A – Diagnostic laboratory
For the investigation of platelet refractoriness, HLA/HPA/HNA antibody screening, HLA/HPA/HNA typing, transfusion-related acute lung injury (TRALI), transfusion associated graft versus host disease (TA-GvHD), post transfusion purpura (PTP), non-haemolytic febrile transfusion reaction (NHFTR), HLA typing & disease association.

3D – Platelet Immunology
For the investigation of fetal/neonatal alloimmune thrombocytopenia (NAIT), heparin induced thrombocytopenia (HIT), other drug related thrombocytopenias, autoimmune thrombocytopenia, thrombasthenias.

3E – Granulocyte Immunology
For the investigation of neonatal alloimmune neutropenia (NAIN), adult autoimmune neutropenia, infant autoimmune neutropenia, drug related neutropenia.
