Ordering HLA Selected Components

Core laboratory working hours: Mon – Fri, 9.00 – 17.00 excluding bank holidays.

New requests for Human Leukocyte Antigen (HLA) selected platelets
Hospital staff must first phone their local H&I laboratory to discuss patient information and testing and component requirements. An order form (FRM 558) will be faxed to the hospital by the H&I laboratory. The completed order form should be faxed back to the H&I laboratory. This form is not available on the website to ensure this essential process is followed.

Second and subsequent orders
For named patients who have previously received HLA selected platelets, please use OBOS (Online Blood Ordering System) to order specialist stock (HLA and HPA products). Orders for emergency requests and planned procedures can be placed via OBOS.

To ensure the best matched product is provided for your patient, please place orders on OBOS for planned procedures at least 24 hours in advance of the required delivery time, Monday to Friday. A poster is available providing guidance on ordering HLA/HPA selected platelets. A pdf version can be downloaded here. Alternatively hard copies of the poster are available from the distribution hub (order code BLC733).

Out of hours requests for HLA selected platelets
HLA selected platelets for the weekend should be planned in advance during normal working hours. The out of hours service is for emergency use only not for routine orders.

Ordering HLA selected platelets when OBOS is unavailable 
If OBOS is unavailable FRM559 (pdf) can be used to request repeat specialist products. Please fax the form followed by a phone call to confirm receipt to:
